Eligibility

Serves ages 6 months and older for flu vaccines. HPV vaccine is for males and females age 12 and older. Other vaccines may have different age limits.

Required documents

Usually none, although may need a doctor's order for some services.

By appointment. Clinic hours - Tuesday: 8:00am-10:30am and 2:00pm-5:00pm. Tuberculosis (TB) screening is available on a walk-in basis M-Th: 8:00am-4:00pm Friday: 8:00am-1:00pm.

Some vaccines and shots - $15.00. Blood pressure screening is free.

Provides blood pressure screenings, tuberculosis (TB) skin tests, and vaccinations or shots for: Seasonal Flu, Adult Tetanus/Diphtheria (Td), Measles, Mumps, Rubella, Hepatitis A and B, Pneumonia, Pertussis (Whooping Cough), Meningitis, Shingles and HPV.
