Ohio residents 18 years and over. Must be currently unemployed through no fault of their own.

Ohio Driver License/State ID. Social security card. Contact information for individual. Contact information and employment dates for employers for previous six weeks. Reason for unemployment. Dependents' names, social security numbers and dates of birth.
