Eligibility
Open to Kentucky residents who are eligible for or currently receiving Medicaid, and also meet at least one of the following criteria:
- are age 65 or older
- have a physical, intellectual, or developmental disability
- have an acquired brain injury
- are ventilator-dependent

Application process
Individuals not currently enrolled in Medicaid must apply and be approved for Medicaid prior to applying for a waiver. Applications for Medicaid may be submitted online, by phone, in person, or by mailing, faxing, or hand-delivering a completed application to the DCBS office in your county of residence.
Waiver applications may be submitted online or in person at an Aging and Disability Resource Center (ADRC) or a Community Mental Health Center (CMHC).

Description
Oversees multiple Medicaid waiver programs for residents of Kentucky who need ongoing support due to their age, a medical condition, or a disability. These programs provide special coverage through Medicaid for services and supports to be provided in an individual's home or community rather than an institutional setting and include the Michelle P. Waiver, Supports for Community Living, Home and Community Based, Acquired Brain Injury (Acute and Long-Term Care), and the Model II Waiver.
