Eligibility

Open to those with household income at or below 300% of federal poverty level. They may be insured, uninsured, or simply have inadequate prescription coverage. May be enrolled in Medicare Part D.

Required documents

Original prescription and the application (which can be completed online or over the phone.) Income documentation is NOT required - the signature on the application or verbal verification is accepted as attestation that the information is true and accurate.

Application process

Can enroll online or by phone. Applications can also be printed off the website and mailed or faxed in. Once approved, medication ships within 7-10 days. Telephone Call Center can assist with any questions.

Fees

Fixed fee (low cost), medication prices vary. There is no charge for shipping and handling.
