Ages 5-18 who are enrolled in the Mayfield City School District which includes Mayfield Village, Mayfield Heights, Gates Mills and Highland Heights.

Application process
Phone child's school to speak with the nurse during the school year. May also phone for information M-F: 7:30am - 4:30pm during the school year, M-F: 8:00am-3:30pm during the summer.
