Eligibility

Individuals age 18 or older.

Required documents

Photo I.D., Proof of income (three pay stubs, previous year's tax return, court agreement for payment), and Medicaid or Insurance card if applicable. Will need proof of residence if requesting sliding scale (a piece of mail with name and current address).

Hours

Program duration averages 8 weeks, but is flexible to accommodate individual needs. Treatment is provided 3 hours a day, 3 days per week, with morning and evening programs available.

Application process

Phone to schedule an intake appointment: M, F: 9:00am-5:00pm; Tu-Th: 9:00am-8:00pm. Clients must complete a detoxification program before starting treatment.

Fees

Offers sliding scale for residents of Lake or Geauga Counties. Accepts Medicaid and private insurers (if they are willing to cover treatment).

Description

Provides a supervised, structured outpatient treatment program for those with substance dependency. Services include: recovery planning, counseling, support and education. Will provide outpatient opioid detoxification if the client is assessed to need it (non-narcotic medication may be administered to treat the symptoms of withdrawal). Clients will be required to also attend AA meetings, or other 12-step community support programs. Support and counseling are offered to family members of the individual in treatment. Can incorporate counseling for dual diagnosis.
