Eligibility

You may be eligible for the SRS program if: • You are a U.S Citizen or have a qualified immigration status; • You have income at or below 150% of the Federal Poverty Level; • You are age 21 or older; • You have been diagnosed with a severe and persistent mental illness, are actively on the solid or soft transplant waiting list or have a diagnosed chronic condition, which includes certain malignancies, HIV/AIDS or immune deficiencies, end stage renal disease (ESRD), sickle cell anemia, cystic fibrosis, hemophilia or if you have had a previous organ transplant; • You need help with medical appointments and/or activities of daily living; • You have been determined to meet the definition of disability used by the Social Security Administration for purposes of SSI or SSDI (this does not apply if you are under 65 with ESRD, or over 65 with certain diagnosed chronic conditions or actively on the solid or soft transplant waiting list); and • You do not live in a nursing facility, hospital or similar setting. If you are eligible, you may get these Medicaid-covered services in addition to your current Medicaid health care coverage: • Recovery Management - Assistance developing a plan of care specific to your needs • Individual Placement and Support (IPS) - Supported Employment - Help finding and keeping a job • Peer Support - Support from others with similar life experiences

Required documents

Agency can look up most supporting documents through an electronic records search - the client will be notified if required to supply any extra supporting documents. If necessary, client may be asked to provide one or more of the following: proof of age, medical proof of disability, proof of medical expenses, proof of income and assets. Applicants who are U.S. citizens and not receiving Medicare or SSI may have to show proof of citizenship. Primary documents for this purpose are a valid or unexpired passport, certificate of naturalization, or certificate of US citizenship. Those who do not have one of these will need TWO of the following documents: a certified birth certificate, hospital birth record, or religious birth record made within 3 months of birth, PLUS an identity document, which in most cases is a government issued ID such as state ID or driver's license. They may also need documentation of name change, if applicable. If documents are unavailable, agency worker will assist the client to obtain them or find other options upon request. Services will NOT be denied or terminated because of this requirement, as long as client makes an effort to comply.

Description

Serves Lucas County residents who are/have been diagnosed with a serious and persistent mental illness, a diagnosed chronic condition, or who are active on the solid organ or soft tissue transplant waiting list may be eligible for the Specialized Recovery Services (SRS) program. Services available through the SRS program include recovery management, individualized placement and support, and peer support. Individuals eligible for the SRS program also receive full Medicaid coverage.
