Eligibility

- Serves adults age 19 to 64 who are not currently receiving Medicare. - Must live in Carroll County and be a United States (U.S.) citizen or legal resident for 5 years. - Financial eligibility is set at 138% of the Federal Poverty Level and is based solely on the taxable income of applicant and their dependents, not household income. For example, since employment, alimony and unemployment compensation are taxable they would be included as income, but child support, Supplemental Security Income (SSI) and workers' compensation are not taxable so they are not included.

Required documents

- One or more of the following, as requested: Proof of age, proof of medical expenses, proof of income and assets. - Applicants who are U.S. citizens will have to show proof of citizenship. Primary documents for this purpose are a valid or expired passport, certificate of naturalization, or certificate of U.S. citizenship those who do not have one of these will need TWO documents: a certified birth certificate, hospital birth record, or religious birth record made within 3 months of birth, PLUS an identity document, which in most cases is a government issued I.D. such as state I.D. or driver's license. They may also need documentation of name change for any reason. - If client cannot afford the necessary documents, or documents are unavailable, agency worker will assist the client to obtain them or find other options. Services will NOT be denied or terminated because of this requirement, as long as client makes an effort to comply.

Application process

- If needed, individuals may call for information from this agency. - Clients may begin the application process by phone, but will be required to mail in supporting documentation following intial call. - The application process may be started online through the Ohio Benefits self-service portal, over the phone, or in person. Arrangements will then be made with agency. - MCP-eligible clients must select and enroll in an MCP, or will be auto-enrolled randomly (plan can be changed with reason, or during open enrollment). - Those who are enrolled in an MCP will receive an MCP identification card, which will replace their monthly Medicaid health card. MCPs will assist with transportation costs for clients who fulfill certain eligibility criteria - clients should contact their plan directly for this information.

No fee to apply. - Once receiving coverage, individual MCPs may charge co-payments for some services.

Description

Provides public insurance to cover medical services that may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. Most clients will have to choose and enroll in (will have their care provided by) a Managed Care Plan (MCP). When it is time to renew, Enrollees will receive a packet, including information regarding Medicaid status and forms for re-certifying coverage.
