HMG Early Intervention-Infants and toddlers age birth to 3 who have a developmental delay or diagnosed condition as defined by rule.

Required documents

Will be requested as needed.

Hours

Mon-Fri 8am-4:30pm. After hours home visits are available based on needs of family

Languages

American Sign Language; Spanish.

In-home services. When you have special concerns... - your child may be showing delays in any area of development - your child has a medical condition which may affect his/her development - you may need access to additional services and supports in Help Me Grow Individualized supports include: - Comprehensive Developmental Evaluation - Connection to therapeutic services - Specialized services based on a child's individual needs - Assistance in transitioning to school-based or other programs. - IFSP - Individualized Family Service Plan
