No restrictions, but only has birth and death certificates for those who were born or died in Lakewood. For late birth registration, serves only those whose baby was born within the city of Lakewood and not in a hospital.

None (agency will supply state forms to be used for late birth registration of babies born in Lakewood but not in the hospital).

Walk in for service. Deputy Registrar will work closely with parents who are filing late registration, to explain all requirements and procedures.

Fixed fee - higher for certified copies.
