Serves residents of Summit County age 60 and over with a household income at or below 130% of the Federal Poverty Level.

Required documents

Must complete an application and provide proof of age (birth certificate, state identification or driver's license) and proof of residency (such as utility bill, state identification, driver's license or so forth).
