Eligibility

-Resident of Lucas County for at least six months -Not eligible for private or public health care -Income at or below the 200% of the federal poverty level -Not eligible for insurance or public coverage -Does not require to have Social Security number -Only those who receive a waiver from the federal Marketplace and a denial letter from Medicaid will be eligible to enroll

-Completed application -Proof of income -Social Security number (if available) -Denial letter from Medicaid and exemption from Marketplace for those who are US citizens or US nationals
