Description

Provides case management for individuals with compulsive hoarding issues for older adults and individuals with disabilities. Offers supportive services such as home care services coordination, assessment, and referral though the Aging & Disability Resource Center (ADRC), contract with SW consult for those on Passport and Ohio Homecare Waiver (OHCW), chore services, and homemaker services through Passport and OHCW. NOTE: Does not offer direct counseling services.

Plans, funds, and coordinates services for older persons and persons with disabilities in Portage, Stark, Summit, and Wayne counties to live with dignity and choices in their homes and communities for as long as possible. Administers PASSPORT, MyCare Ohio, The Assisted Living Waiver, The Ohio Home Care Waiver, Care Coordination, Elder Rights, Family Caregiver Support Program, Nursing Home Placement Assistance Program, and other services designed to help older adults and individuals with functional limitations maintain independence. Provides funds for services administered by other community agencies including client finding, specialized information and referral, transportation, home-delivered meals, medical treatment, personal care, respite care, safety monitoring, health assessments, congregate meals, recreational activities, and adult day care.
