Eligibility
Individuals, age 18 and older, recovering from substance use disorders and/or diagnosed with a mental health disorder.
Required documents
State ID, proof of income, proof of residency, proof of insurance, birth certificate, Social Security number.
Hours
By appointment.
Application process
Phone 24 hours a day, 7 days a week for information or M-F: 9:00am-6:00pm to make an appointment.
Fees
Services are billed to Medicaid and Medicare. All MCOs and managed care plans are accepted. Additional funding is available for those without insurance.
