Eligibility

Serves ages 18 and older. Must be medically stable and require a minimum of two therapy disciplines (some combination of physical therapy, occupational therapy, and/or speech therapy).

Health insurance card, if applicable or proof of income if uninsured.

Offers two programs. One is M, W, F: 9:00am-4:00pm. The second program is offered T and Th: 9:00am-4:00pm.

Application process

Anyone can call for information or an appointment M-F: 8:00am-4:30pm. Physician's order will be needed for service.

Fees

Accepts most insurance.Third party payment accepted, including Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Those living further than 150 miles from the main site may have other requirements.

Description

Provides a program for those with neurological and non-neurological impairments who no longer require 24-hour inpatient medical care but who still require intensive rehabilitative therapy (instruction in activities of daily living, for example) and occasional nursing. A meal is included in the day's activities.
