Must be a U.S. citizen. May receive 3 card replacements per calendar year and 10 in a lifetime.

Required documents

Photo I.D. (not expired) is mandatory for an original or replacement card, and for most applicants this will be sufficient. For anyone whose proof of age and U.S. citizenship is not already registered with the SSA, at least one additional document must be provided to verify this information. There are three categories of proofs (identity, age, and U.S. citizenship), and a single document may be used for two different categories, but not all three. PROOF OF IDENTITY: U.S. driver's license, U.S. passport, or state-issued I.D. card. May also accept photo identification issued by the U.S.. military, current employer, health insurance company, or school. To replace a child's card, a birth certificate is not sufficient proof of identity (though satisfactory for proof of age and citizenship). Possible proofs of identity for children in addition to the previously listed documents may include adoption decrees, medical records, daycare center records, or religious records. Furthermore, parents of children seeking Social Security cards will need to bring their own valid photo I.D. PROOF OF AGE: U.S. birth certificate is mandatory, if it exists. If not, may accept a religious record made before age 5 showing date of birth, U.S. hospital record of birth, or U.S. passport. PROOF OF U.S. CITIZENSHIP: U.S. birth certificate, U.S. consular report of birth abroad, U.S. passport, Certificate of Naturalization, or Certificate of Citizenship. For name changes: will need documents to prove your identity, support the requested change, and establish the reason for the change.

Application process

Visit website for more information and to download the application. Will need to mail application with all required documents. Documents must be either originals or certified copies by the issuing agency. Does not accept photocopies or notarized copies of documents. Any documents mailed will be returned along with a receipt. Will have to wait approximately 7 days until it comes in the mail: no printouts will be given. Born outside the hospital: will first need a birth certificate and then can apply for the social security number at a local branch, can take up to 3 months.
