Disabled workers who are no longer capable of substantial work. Must be fully insured under Social Security to be eligible. Other eligibility requirements may apply.
Required documents
Completed application. Will be advised about required documents. Generally: Social Security Card, Birth certificate, medical records, names, addresses and phone numbers of all medical professionals that has assisted client, summary of work history and most recent W-2.
