Individuals diagnosed with severe persisitant mental illness or developmental disability and active client of Mahoning or Columbiana County Mental Health and Recovery Board, contracted agency, or by Mahoning, Columbiana or Trumbull Boards of Development Disabilities.

No public referrals. Present case to clinical supervisor of MH/DD agencies for referral to program.

Description

Guardianship program for severely mentally disabled persons who are identified by the mental health/developmental disabled systems for services. All referrals must go through clinical issues meetings or the case manager supervisor.

Provide Crisis Intervention and Suicide Prevention services to Mahoning, Columbiana, Trumbull and Ashtabula Counties; 24/7 2-1-1 Information and Referral access for Columbiana, Mahoning, and Trumbull Counties and after hours for Ashtabula and Lake Counties; after hours link to various agencies in Mahoning, Columbiana, Trumbull and Ashtabula Counties; support groups for Survivors of Suicide; Senior Line; housing assistance and payee programs; Intensive Guardian Program; Recovery Services (Mahoning and Trumbull counties); Victim's Assistance; Community Center (Mahoning and Columbiana Counties); Special Navigator for Families with Special Needs.
