Eligibility

Must be a woman who is pregnant OR lactating with a baby up to one year of age, OR be postpartum for up to six months after the termination of a pregnancy by birth, miscarriage, or abortion. Those younger than age 18 do NOT need parental/guardian permission. Children are eligible up to fifth birthday. Eligible individuals must live in Lucas County, must be at nutritional risk as determined by an agency professional using the Federal Nutritional Risk Priority System and must meet income guidelines. Income eligibility guidelines are up to 185% of the Federal Poverty Guidelines that were in effect the previous April 1. Number in household includes unborn children. Definition of income does NOT include household assets. Individuals receiving Medicaid, Ohio Works First, or Food Stamps automatically meet income eligibility.

Required documents

Photo ID that shows name and birth date for those in the household who are applying for benefits. Could even be a crib card from the hospital. Need proof of pregnancy from doctor or clinic, unless the pregnancy is clearly visible. Also need 3 pay stubs, SSI statements, proof of Medicaid, Ohio Works First, Food Stamps or other means of verifying income, a written statement from child's father saying he is paying child support, proof of child's current immunizations, proof of residency (including utility, telephone or credit card statement).

Description

Provides supplemental food vouchers; nutrition education; breastfeeding education, support and counseling; and referral services to eligible women and children. During the summer, also provides a one-time $15 supplemental voucher to each client for fruits and vegetables from selected local growers (Farmers' Market Nutrition Program) - these are available while supplies last. Also offers speakers who discuss the program and the value of good nutrition.
