Provides a link to needed supports and to coordinate services. Assesses individual needs, facilitates development of individual service plans, arranges for services and monitors the provision of those services. Assists families with questions, planning or referrals on subjects such as Social Security, guardianship, Medicaid, waiting lists, etc.
