Anyone diagnosed with or suspected to have one of the 43 neuromuscular diseases in MDA'S program.
Required documents
Agency will advise, may need diagnostic statement from primary physician. Health Insurance card (if applicable).

Fixed fee; Primary insurance is billed first, and MDA picks up most balances and co-pays.
