Eligibility

Individuals 18 years of age and older who are living with HIV/AIDS and their families/partners living in Cuyahoga, Ashtabula, Geauga, Lake, or Lorain Counties.

Required documents

Physician referral is not required, but must already have had a positive HIV test.

Application process

Call M-F: 8:00am-5:00pm to make an appointment. Appointments are available without any substantial delay.

Fees

Accepts private insurance, Medicaid, and Medicare. For those with no insurance, care and medication assistance can be provided through Ryan White Title I Emergency Assistance Program.

Provides comprehensive care for those with HIV/AIDS and their families or partners. Provides both treatment and case management. The treatment and support team includes physicians, nurses, pharmacists, and social work case managers (case management is automatic).
