-Parents/guardians will need photo ID for themselves. Proof of custody/guardianship. -For all registrations: Birth certificate, passport, Green Card or I-94. Proof of Address (utility bill, except telephone bills and cable bills). Proof of custody or guardianship. Emergency contact information and preferred doctor/hospital. -Changing schools in district: Withdrawal papers or last grade card. IEP/MFE if applicable. -Transfer to district: Name and address of last school attended. Withdrawal papers or last grade card. Immunization records. IEP/MFE if applicable. -Entering kindergarten: Immunization records. Developmental details. Health information.
