Open to anyone who needs assistance with these issues.

Required documents

Photo I.D required. For sliding scale, copy of current Federal Income Tax return (not W-2's), or proof you did not file, or proof you asked for a filing extension proof of gross income for the past 12 months. Proof of Family Membership including birth certificates of minor children, marriage certificate/divorce decree/separation statements. Proof of Address (current utility bill in your name, current voter registration card, or current legal mailing in your name). Proof of Residency (Resident Alien Card/Permanent Resident Card for all family members, Immigration & Naturalization Service Affidavit of Support verifying sponsorship, and U.S. Passport for all family members or citizenship documentation for all family members).

Fees

Services are on a sliding scale, and may be free for patients who meet income requirements.
