Required documents
Child’s insurance card at each visit.
Hours
8am-4pm Mondays. First Monday of each month, immunizations are available from 8am-6:30pm to accommodate working parents. No appointment is necessary. The Ohio Department of Health does still offer vaccinations for children with no insurance.

Fees
Medicaid and most insurances are accepted. Medicare Part B is accepted for Influenza and Pneumonia Vaccines.
