Eligibility
- Serves residents of the city of Medina, Lodi, Chippewa Lake, Medina Township and Montville Township.
- Clients may receive financial assistance once per year.
Required documents
- Proof of income for all assistance programs.
- Cost of rent for rental assistance.
- Proof of shut-off notice for utility bill payment assistance.
- Proof of existing prescription refill with a doctor's signature.
- Must show photo ID when picking up a check for payment assistance.
