Eligibility

Age 18 and older, with intellectual and other developmental disabilities in Cuyahoga County. Primary focus is on those with moderate, severe, or profound mental retardation.

Required documents

Health insurance card Medicaid card need completed packet of information from the CCBDD, including physical and physician's diagnosis.

Application process

Intake is through the Cuyahoga County Board of Developmental Disabilities (CCBDD). Phone the CCBDD M-F: 8:00am-4:30pm for intake and to be placed on their priority list. May phone Northeast Care Center M-F: 9:00am-5:00pm for information or to arrange a tour.

Fees

Fixed fee; However, clients' fees paid mostly by Medicaid or through the Cuyahoga County Board of Developmental Disabilities.

Provides group residences for adults with intellectual disabilities and developmental disabilities.
