Eligibility
Must be a student or teacher of the Wickliffe City School District.
Required documents
I.D required. Medicaid insurance card. More documentation may be requested for district verification such as mail, or rental lease.
Hours
Every other Tuesday: 3:00pm - 6:00pm. Call ahead to schedule an appointment and available appointment slot options will be provided.

Medicaid is accepted, however, services will be covered by the WFRC through a grant provided by the Cleveland Foundation until funds are no longer available.
