Eligibility

Open to those ages 18-24 in Cuyahoga County who meet the HUD definition of chronic homelessness and have been diagnosed with a major mental illness such as schizophrenia, bipolar disorder, severe depression, or substance abuse issues. Must be able to live independently and safely without 24-hour supervision.

Required documents

Documented history of psychiatric treatment. Proof of homelessness according to the HUD definition written referral.

Referral from Case Worker required.

Fees

Fees are 30% of income for rent and nominal charges for utilities and phone; most clients receive SSI for mental disabilities, case worker will help those who do not receive SSI apply.

Offers scattered site housing (up to 23 clients). Individual and group programs, designed to provide intensive skill training in independent living skills, are available to residents.
