Eligibility

All services must be physician ordered. Medicare coverage requires the patient to be essentially home-bound and needing skilled home care services. Medicaid does not require either home-bound status or skilled need. MyCare Ohio must pre-authorize care.

The Hospice team of Nurses, Social Workers, Aides, Clergy and Volunteers helps patients to live their remaining days with dignity, comfort and control. Family members have bereavement support for 13 months after the death of the patient. The Home Health team of Nurses, Aide and Therapists focus on rehab and long term in-home care.
