Patients of the health center that are uninsured, underinsured or insured but have problems paying their medical bill.

Required documents

Those planning to request financial assistance services will need to provide proof of family income, complete a Financial Aid Application, and undergo financial screening.

Application process

Phone for information. Generally patients without third-party payment, including Medicaid and Medicare, are not scheduled for appointments until they meet with a Financial Counselor. Bill payment assistance may be applied for before or after appointments.

Fees

No fee for service; Offers free and discounts on medically necessary services, based on income up to 400% FPL. Those who have no health insurance will be screened for free care or discounts based on income. Patients with income levels up to 200% of the Federal Poverty Level (FPL) qualify for free care based on the State of Ohio Hospital Care Assurance Program (HCAP) or the Financial Assistance Policy. Balances over 200% of the FPL will be discounted based on Amount Generally Billed (AGB)%.
