Application process
An assessment is required prior to initially receiving service. Phone to schedule an assessment M-F: 8:00am-5:00pm. If assessment results in individual being eligible for service, will give number for call center to schedule transport. Eligible individuals may make reservations for transport from seven days before the needed trip up to 12:00noon the day before the trip.
