Adults 21 and older who reside in Eastern Cuyahoga County.

Required documents

Proof of insurance (if applicable) Referral from a current physician for physical, occupational, or speech therapy.

Application process

Phone M-F: 8:30am-5:00pm to schedule an on-site intake appointment. Referral required from a current physician for physical, occupational, or speech therapy. Once an evaluation has been conducted, it must be approved by the client's physician before appointments will be scheduled.

Fees

Fixed fee; third party payment accepted. Ancillary services (such as meals, grooming, etc.) and art and music therapy are not covered by most third party insurance and are therefore billed directly to the clients on a monthly basis.
