Eligibility

Serves Cuyahoga, Medina, Lake, Lorain, and Geauga counties, although service in those counties is subject to whether facilities located there accept the waiver and are certified. Must be aged 21 and older and must either reside in a nursing home, be enrolled in a Medicaid waiver program (PASSPORT or Ohio Home Care), or be a resident of an assisted living facility in Ohio for at least six months, whose personal finances have been depleted (making them eligible for Medicaid). Applicants must have a minimum monthly income (agency will review amount) since participants are expected to pay room and board costs established by the state. Must be able to live in a room alone and be able to live independently enough for intermediate level of care.

May not need to provide additional documentation - any exceptions will be explained at intake.

Hours

Residence 24 hrs a day, 7 days a week office hours M-F: 8:00am-5:00pm.

Application process

Call Intake M-F: 8:00am-5:00pm for phone screening and to schedule an assessment. Staff will complete any necessary paperwork to change Medicaid programs.

Fees

Resident must pay fee for room and board, up to a minimum amount that changes yearly. If monthly income is higher than required minimum for room and board, individual will be required to pay additional fees to the facility for services. Medicaid pays the facility any remaining costs for care, and allows a small amount to the resident for personal cash expenses.

Description

Provides funds to cover the cost of care in an assisted living center for those who meet eligibility and require a residential setting but not the services of a nursing home. (Does not cover medication, co-pays, or services such as a visiting nurse.) Assisted living facilities must have agreed to accept this reimbursement, and must be certified by this agency. Participants pay standard room and board costs.
