Individual(s) diagnosed with amyotrophic lateral sclerosis (ALS) and their families.

Required documents

No requirements for general consultation and support. Verification of Diagnosis is needed to access some Chapter programs.

Hours

M-F: 9:00am-5:00pm. Books and videos may be borrowed for up to four weeks.

First visit website to fill out form, once form is filled a ALS care team member will reach out. May also phone during service hours for more information.

Healthcare professionals specializing in amyotrophic lateral sclerosis (ALS) provide assistance and education regarding disease progression, management of care needs, and coordination of services. Regional Care Coordinators are available to help identify the needs of patients and their families. Care Coordinators can offer guidance to find or develop appropriate solutions, support or services.
