Call M-F: 8:00am-4:00pm for information or to make an appointment. Clients can self-refer, but if recently hospitalized it is recommended that a referral come from the hospital social worker, who can more easily provide necessary documentation.
Fees
Accepts Medicare or private insurance, depending on circumstances. There are provisions for patients without financial resources or insurance - phone for full information.

Description
Provides Medicare certified and private duty home health care services. Will work with the primary physcian for medication management, post operative care, wound care, diabetic teaching, COPD and catheter care, and other skilled needs. After skilled services have ended may assist with meal preparation, light house keeping, medication reminders, bathing, and providing companionship. Also provides Palliative care - a multidisciplinary approach to specialised medical care for people with serious illnesses. It focuses on providing patients with relief from the symptoms, pain, physical stress, and mental stress of a serious illness—whatever the diagnosis. The goal of such therapy is to improve quality of life for both the patient and the family.
