Phone M-F: 8:00am-4:30pm or check website to download and print the form. Walk in M-F: 8:00am-4:30pm or mail the form with payment. Birth and death certificates can be ordered online at https://rossoh.permitium.com/rod. Requests by mail must have a money order enclosed. Requests are generally processed in 1-2 days.
