Eligibility

Serves full-time residents of Cuyahoga Falls age 65 and over who have no able-bodied person residing in the home and/or income qualifying residents with a doctor verified disability. - Driveways must be concrete or asphalt. - Service is not available to multi-unit residences. - House numbers must be clearly visible from the street. - NOTE: Residents may use the vouchers at their own discretion and must utilize the approved contractor.

Required documents

Must complete an application and provide supporting documentation including: - Proof of Homeowners Insurance. (The current declarations page. Not the entire policy.) - Current photo ID (license or state identification card) or birth certificate. - Letter from applicant's physician documenting their disability if 64 years old or under.

Applications due each year by the last Friday in October by 4:00pm. May end early if program is full.
