Eligibility

Serves residents of Cuyahoga County age 16 and older who are U.S. citizens or qualified aliens with income at or below 250% of the Federal Poverty Guidelines. Must be determined disabled by the Social Security Administration or by Ohio Medicaid. Must be employed in paid, taxed work (full- or part-time). Must meet certain financial criteria (there are a number of deductions, so anyone who may otherwise be eligible is encouraged to apply). Premiums are required for those with annual gross income greater that 150% of Federal Poverty Level (FPL). Consumers who must pay premium will be subject to termination from the program if they do not pay the premium for two consecutive months. However, they may re-enroll once all delinquent premiums are paid (as long as they meet all other eligibility).

Application process

Clients may begin the application process, renew, and/or update their case information by calling the Eligibility Contact Center: M-F: 8:00am-1:00pm (due to high call volume, caller may be given the option to receive a call back - if they choose this option they should get a call within 1-2 business days). May also apply, renew, and/or update case information through website anytime (client must have an email address to do this).

Fees

Application to the program is free. Clients earning over a certain amount may be required to pay a monthly premium which is deducted from the client's Social Security Income.
