Eligibility

Serves Summit County residents who are homeless or at risk of homelessness, have severe and persistent mental illness, or chronic substance abuse, and are not currently engaged in treatment.

Call the office for more information. Most referrals come from area churches, social service agencies, law enforcement, and healthcare providers.

Description

Identifies and attempts to introduce individuals who are homeless and experiencing mental illness or substance use disorders into the social services/ behavioral healthcare service delivery system. Services include case management and referrals to other supportive services. The Homeless Outreach Center offers showers, laundry facilities, and a telephone for homeless individuals to use.

Provides community-based behavioral and physical healthcare services in Summit County as well as housing support, homeless outreach, and supported employment. Serves as a safety net for those who suffer from illnesses such as schizophrenia, bipolar disorder, major depression, and personality disorders. Services include psychiatric and medical programs, rehabilitation, advocacy, and support for recovery. Agency is a Certified Community Behavioral Health Clinic (CCBHC).
