Eligibility

Caregivers who are providing care for a relative age 60 or older, or caregiver 60 or older caring for a family member 18 or younger who is developmentally disabled.

Description

Assist caregivers of an older relative who has a serious chronic illness or disability. Services include: information, education, supportive counseling, training, respite services, on-going care coordination. Respite services may include: adult day care, personal care and homemaker/chore services.
