- Call the office or visit the website for more information. - All applications must have a street address; PO Boxes cannot be accepted. A sole owner is required to furnish their social security number or federal tax identification number. If the company is a corporation, LLC or LP, list the Ohio Corporate Charter number, Ohio Foreign License number, or Articles of Organization number issued by Ohio's Secretary of State. Also needed are the names and addresses of the corporate officers.

Fixed fee. The annual fee for a vendor license (ST-1) is payable by check or money order.
