Eligibility
Total household income at or below 200% of the Federal Poverty Guidelines. Households with children must have a denial from the PRC program to qualify.

* 30 days proof of income for all household members
* Proof of automobile insurance
* Proof of automobile registration
* Automobile title or payment book
* 2 quotes from ASE (Automotive Service Exellence) repair shops that accept purchase orders
* Proof of co-payment (if applicable) for repair costs that exceed the maximum benefit amount
