Eligibility

Covers children age birth up to age 19 in families earning up to 206% of Federal Poverty Level, and pregnant women earning up to 200% of Federal Poverty Level. Coverage includes the post partum period. Must live in Cuyahoga County and be a citizen or legally residing non-citizen. Families earning up to 156% of Federal Poverty Level are permitted to have other insurance, and Medicaid will always be the payer of last resort. Legally residing non citizens may have additional requirements to qualify.

Required documents

Agency can look up most supporting documents through an electronic records search - the client will be notified if they need to supply any extra supporting documents. If necessary, the client may need to provide: proof of Income Applicants who are U.S. citizens and not receiving Medicare or SSI will have to show proof of citizenship (primary documents for this purpose are a valid or expired passport, certificate of naturalization, or certificate of U.S. citizenship) those who do not have one of these will need TWO documents: a certified birth certificate, hospital birth record, or religious birth record made within 3 months of birth, PLUS an identity document, which in most cases is a government issued ID such as state ID or driver's license. They may also need documentation of name change for any reason. If client cannot afford the necessary documents, or documents are unavailable, agency assist the client to obtain them or find other options. Services will NOT be denied or terminated because of this requirement, as long as client makes an effort to comply.

Hours

M-F: 9:00am-4:00pm. Agency goal is to provide approval or denial (original or reinstatement) within 45 days, but in some cases, determination may take longer.

Application process

Clients may begin the application process, renew, and/or update their case information by calling the Eligibility Contact Center: M-F: 8:00am-1:00pm (due to high call volume, caller may be given the option to receive a call back - if they choose this option they should get a call within one business day). May also apply, renew, and/or update case information through website anytime (client must have an email address to do this). No interview is needed. Agency can look up most supporting documents through an electronic records search - the client will be sent a checklist for any missing supporting documents. Once an eligibility determination is made, a notice of action (NOA) is mailed to the applicant. Applications are available and may be dropped off at any Neighborhood Family Service Center (NFSC) M-F: 9:00am-4:00pm. Free scanning is available at the Virgil Brown Building M-F: 9:00am-4:00pm (drop box at this location is available 24 hours a day). Completed application may also be faxed (number is on the application). Free faxing is available at branches of the Cleveland, Cuyahoga, and Shaker Heights Libraries during their normal hours of business. MCP-eligible clients must select and enroll in an MCP, or will be auto enrolled randomly (plan can be changed with reason, or during open enrollment). Those with general questions about Medicaid, Medicaid Managed Care Plans, or who simply need to change their address/contact information can call the Medicaid Customer Service Hotline M-F: 7:00am-8:00pm; Sa: 8:00am-5:00pm. Starting March 28th, 2025: Clients using the online benefits portal will need to either create or sign into an OHID account. After they have created their OHID and logged into it, they will be asked to set up Multi-Factor Verification (a security feature to protect against identification theft). Those who need help navigating the set up process can phone the Contact Center M-F: 8:00am-4:00pm.

Description

Provides health insurance through a managed care plan (MCP) for children and pregnant women in low-income families. Services for children include hospital or acute illness care, well-child checkups, limited dental care, eyeglasses, prescribed medication, and immunizations. For pregnant mothers, will cover doctor visits before and for 60 days after delivery, prescriptions, and the delivery itself.
