Must get referral from primary physician and go through hospital's admitting process before calling to schedule appointment for audiological evaluation. Call M-F: 9:00am-5:00pm for an appointment.
Fees
Accepts most insurance.Third party payment accepted, including Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Those living further than 150 miles from the main site may have other requirements.
