Must be Medicaid eligible.

Required documents

Demographic assessment, individual treatment plan, service level notes, fee agreement and receipt of client expectations, rights and grievances. Also infection control information and release of information forms.

Hours

Mon, Wed, 9 am-8 pm. Tue, Thu and Fri 9 am-5 pm.

Application process

Accepts referrals through the Adult Parole Association and the Department of Youth Services. Other referrals are also considered providing the client is Medicaid eligible.

Medicaid approved fees.
