Currently employed Cuyahoga County residents. Annual household income MUST be between 200% and 300% of FPL. Up to 2 adults per household may apply (if 2 adults applying, both must be employed) A household can receive assistance once per lifetime. This is a program of last resort. Lost cards cannot be replaced. If other services are available to assist, those referrals should take precedence. Applicants must be willing to release basic personal information to the Cuyahoga County Department of Jobs and Family Services (more information about this release will be provided by a Tier 2 Specialist once initial application is complete).

Photo I.D. required for recipients at the time of pick-up. If picking up for two adults, address on I.D.s much match or, if not matching, must provide current official piece of mail (utility bill, rent invoice, etc.) with matching address in the name of the non-matching adult. More documents may be required during the application process, including proof of employment. Client will be advised what additional documents will be required and how to provide them.

24 hours, 7 days a week to begin application process. Once application process is started follow-up calls and one in-person meeting to pick up the gift card.
