Eligibility

Adults 18 and over with a substance abuse addiction.

Required documents

Proof of residency (can be at least one of the following): piece of mail adressed to client (cannot be a homeless shelter), automobile registration, or a tax return. Will also need 3-4 pay stubs as proof of income (which also can be used as proof of residency) and a medical card (if applicable).

Application process

Phone for an assessment, M-F: 5:30am-3:30pm. Can also walk-in M-F: 6:00am-3:00pm for evaluation.

Fees

Sliding scale. Third party payment accepted. Medicaid accepted. No one will be refused service due to inability to pay. Fee structure differs for out-of-county residents.

Description

Offers chemical dependency assessment and comprehensive outpatient treatment. Treatment also includes full medical screening and drug testing (urine and blood analysis). Also offers testing for Tuberculosis (TB), Sexually Transmitted Diseases (STD's), and Hepatitis A, B and C.
