Eligibility

Referral from local agency, need for identification to secure employment or housing, or ALICE status.

Required documents

If requesting a Drivers License or Permit, please be advised that the BMV will require additional documents to verify Ohio residency, social security number, and address.

Hours

Mon-Thu 8am-5pm Fri 8am-12pm. Recommended to call ahead before arriving.

Application process

Call 740-345-6685 for information. Participants must complete a brief application. Those requesting birth certificates must have been born in Ohio, and must provide their location of birth, mother's full maiden name, and father's full name. Drivers license requests are limited to unrestricted licenses only, UWLC can not pay for fines or reinstatement fees.
