Application to the program is free. Monthly premiums are required for those eligible for MBIWD with an annual gross income greater than 150% of the federal poverty level (FPL). To obtain and maintain health coverage, the full amount of the premium must be received by the due date or it will be considered non-payment. If premiums go unpaid after two consecutive months, then they will be subject to termination from the MBIWD Program. However, they may re-apply and re-enroll into the MBIWD Program once all delinquent premiums are paid (as long as they meet all other eligibility criteria).
